The Washington State Medical Marijuana Authorization Agency LLC is a Seattle based company. We have many doctors who can help you become a medical marijuana patient or renew your authorization for the last time. First things first, click on the contact page and E-mail us your name, E-mail and or phone number. We want to be able to reach you. Next, we need to know if you qualify for an authorization. If you feel your condition meets the requirements, all you need to do is click on the forms page and fill out the Patient Intake form and Risks of Marijuana form. They are needed in order to see the doctor. After that, fax your medical record to (480) 247-5729. Once we receive your information we will create a file for you, and call or E-mail you to schedule an appointment. call us at (206) 979-9-420 if you have any questions concerning the process.
What Medical Records Are We Looking For And How Do I Get Them?
We need to get a copy of your medical record which states your qualifying conditions. This includes any relevant information related to the qualifying condition. In most cases we would like to see at least two or more years of medical history. We do not need your entire record. Just call your doctor’s office and have them fax your records to us. You will have to fill out a release form for them to forward your medical record to us.
This process is quick and easy. The authorization visit is 10-15 minutes, and you will have your authorization when you walk out the door. Our lifetime authorizations are $300. We accept cash and debit. This is everything you will need to get started. If you know of anyone else interested in obtaining an authorization please feel free to give them our business information. If they have a qualifying condition which is not permanent, the fee is $200 for a two year authorization. We look forward to hearing from you.

i am aspiring to be a writer/comedian, who isn't scared to poke fun at the masses of intelects. hear the one about hilary clinton? she came home to find three bears sitting on her couch, she asked where they came from, they replied bill kicked them out of their own home.if thats not funny she went to bed and found three pigs and pinochio playing cards on their bed. before she can say anything, a pig asks, you found his stash again didn't you?( ba ding pow!)
magic there's nothing like it is there?, i performed once, wasn't the experience i planned. see i didn't have the furry rabbit, i had a couple hamsters who look similar but since i own them i can tell them apart. i used only a single hamster in the act. anyways my act was going according to plan. my hamster would squeak and cheer at he end of the act. i brought forth the hamster, and i had the hamster with a medical condition, i guessed wrong. he looked about the room and fell to his side and twitched awful. screams came HES NOT ALL THERE! ITS WITCHERY!
and then the hidden hamster started squeaking and cheering. it seemed as if the twitching hamster was throwing it's voice in a happy manner and still twitching. like a scene out of a stephen king flick.
